Covenant Behavioral Health Residential Facility LLC (Covenant BHRF) is licensed to provide Respite Care and Mental Health Care Services under the AHCCCS program. These services are delivered in a safe, supportive, and therapeutic residential environment in Laveen Village, Arizona that promotes recovery, independence, and community integration.

Who We Serve

Covenant BHRF provides residential behavioral health services to individuals who need a safe, structured, and supportive environment. Our programs are specifically designed for:

  • Individuals living with depression, anxiety, bipolar disorder, schizophrenia, trauma, and other mental health challenges
  • Individuals who need behavioral health treatment, coping skills training, and support for co-occurring disorders
  • Individuals and families who need short-term respite care and relief for primary caregivers
  • Individuals referred by AHCCCS-registered providers, Access Points, health plans, or case managers

Admission Criteria

Clients must meet the following criteria for admission to Covenant BHRF:

  • Referral by an AHCCCS-registered provider or Access Point
  • Need for Respite Care or Mental Health Care Services
  • Medical stability as determined by a licensed healthcare professional
  • Ability to benefit from services provided in a residential setting
  • Agreement to comply with facility rules and participate in treatment planning

Outside Our Scope

While we strive to serve as many individuals as possible, the following situations fall outside our current scope of services:

  • Individuals requiring acute inpatient psychiatric hospitalization
  • Individuals requiring 24-hour skilled nursing care
  • Individuals presenting immediate danger requiring emergency intervention

Our Professional Team

Covenant BHRF maintains qualified staff to ensure quality care and compliance with AHCCCS standards. Our team includes licensed therapists, a psychiatrist / nurse practitioner, behavioral health technicians (BHTs), case managers, peer support specialists, and medical, support, and house staff.

All staff receive initial and ongoing training in AHCCCS regulations and compliance, client rights, trauma-informed care, crisis prevention and intervention, HIPAA and confidentiality, medication administration, documentation standards, and cultural competency. We coordinate care with AHCCCS health plans, primary care providers, behavioral health providers, and families to ensure seamless, integrated care for every individual we serve.

Quality Improvement

Covenant BHRF is committed to continuous quality improvement in all aspects of service delivery. Our Quality Improvement Plan includes:

  • Regular review of client outcomes and satisfaction surveys
  • Incident and complaint analysis
  • Staff training and competency evaluations
  • Policy and procedure reviews and performance improvement initiatives
  • Quality improvement meetings held monthly

Compliance & Safety

We maintain compliance to ensure quality care and protect the rights of our clients through:

  • AHCCCS requirements and Arizona Administrative Code (A.A.C.)
  • Internal and external audits and AHCCCS reviews
  • Regular safety inspections and emergency preparedness drills
  • Fire extinguishers, smoke detectors, and evacuation plans
  • Secure facility entrance and clean, well-maintained living spaces
