Your Rights
As a client of Covenant BHRF, you have the right to:
- Be treated with dignity and respect
- Receive services in a safe and supportive environment
- Confidentiality of all personal and health information
- Participate in the development of your treatment and care plans
- Refuse treatment, except as otherwise provided by law
- Freedom from abuse, neglect, and exploitation
- Access grievance procedures without fear of retaliation
- Receive information about services and costs
- Privacy, and the right to be involved in your treatment planning and decisions
- File a complaint without fear of retaliation, and be informed about your services, benefits, and providers
Your Responsibilities
To help us provide the best possible care, clients are expected to:
- Participate actively in treatment and recommended activities
- Follow facility rules and staff directions
- Treat staff and others with respect
- Take medications as prescribed
- Keep personal information updated
- Maintain the safety and cleanliness of the environment
- Refrain from the use of illegal substances and alcohol
- Report concerns or issues appropriately

Grievance Procedure
Clients have the right to voice concerns or file a grievance without fear of retaliation. Covenant BHRF takes your concerns seriously and is committed to resolving all grievances promptly and fairly. Steps to file a grievance:
Discuss
Discuss the issue with staff or the Administrator. Many concerns can be resolved right away.
Submit in Writing
If unresolved, submit a written grievance form. Staff can assist you with writing your grievance if needed.
Investigation
The Administrator will investigate your grievance within 5 business days, thoroughly and impartially.
Written Response
A written response will be provided within 15 days outlining the findings and any corrective actions taken.
AHCCCS Appeal
If dissatisfied with the resolution, you may contact AHCCCS (Arizona Health Care Cost Containment System) directly.
