Covenant Behavioral Health Residential Facility LLC accepts referrals from AHCCCS-registered providers, Access Points, health plans, case managers, and other community providers. Our streamlined admission process is designed to connect individuals with respite care and mental health residential services as quickly and smoothly as possible.
Our Admission Process
From first contact to welcome day, here is exactly what to expect:
Initial Contact
Call 623-330-7509, email referrals@covenantcareaz.com, or submit the referral form below. We are available to answer questions and walk you through the process.
Pre-Screening
We gather basic information and complete a needs assessment to determine whether our services are the right fit.
Clinical Assessment
Our professional team completes a comprehensive clinical evaluation of the individual's behavioral health needs, strengths, and goals.
Admission Approval
The referral is reviewed and admission is approved based on clinical criteria.
Welcome & Orientation
The client is introduced to staff and the facility, the personalized care plan and safety plan are started on day one, and services begin.
Admission Criteria
Clients must meet the following criteria for admission to Covenant BHRF:
- Be referred by an AHCCCS-registered provider or Access Point
- Require Respite Care or Mental Health Care Services
- Be medically stable as determined by a licensed healthcare professional
- Be able to benefit from services provided in a residential setting
- Agree to comply with facility rules and participate in treatment planning
Documentation Required
Please have the following documentation ready when making a referral:
- Completed referral form
- AHCCCS eligibility verification
- Comprehensive behavioral health assessment
- Current medication list
- Consent for treatment and release of information
- Referring provider information (name, organization, phone, email)
What Happens After Admission
Individualized Treatment Plan
A collaborative, strengths-based treatment plan is developed with the client, including goals, interventions, measurable outcomes, and review schedules. Plans are reviewed at least every 90 days, when needs change significantly, or at the client's request.
Safety Plan From Day One
Every client completes an admission assessment and works with staff to create a personalized safety plan, which is reviewed together, signed, and updated as needs change.
A Full Care Team
Licensed therapists, a psychiatrist / nurse practitioner, behavioral health technicians, case managers, and peer support specialists surround each client with 24/7 support.
